Diphtheria, Tetanus, and Pertussis (DTaP) VIS

5 doses REQUIRED upon school entry.

Polio VIS

4 doses REQUIRED upon school entry Kindergarten and greater. Early Childhood Program Operated by a School for children ages 4 Years and Under: 3 doses REQUIRED.

MMR (Measles, Mumps, & Rubella) VIS

2 doses REQUIRED upon school entry Kindergarten and greater. Early Childhood Program Operated by a School for children ages 4 Years and Under: 1 dose REQUIRED.

Chickenpox (Varicella) VIS

2 doses REQUIRED upon school entry Kindergarten and greater. Early Childhood Program Operated by a School for children ages 4 Years and Under: 1 dose REQUIRED.

Varicella (chickenpox) vaccine is NOT required if child has had chickenpox disease and disease is DOCUMENTED by a physician signature. Without a physician signature, vaccine is required even if you believe child has had chickenpox.

Hepatitis B VIS

3 doses REQUIRED upon school entry.

Tdap (Tetanus, Diphtheria, Pertussis) VIS

All 7th - 12th graders are required to have ONE Tdap regardless of the interval since the last dose of Td (tetanus/diphtheria).

Hepatitis A VIS

Early Childhood Program Operated by a School for children ages 4 Years and Under: 2 doses REQUIRED.

Haemophilus Influenzae Type b (Hib) VIS

Early Childhood Program Operated by a School for children ages 4 Years and Under: 4 doses REQUIRED.

Pneumococcal Conjugate (PCV13) VIS

Early Childhood Program Operated by a School for children ages 4 Years and Under: 4 doses REQUIRED.

Inactivated Influenza VIS

Influenza (flu) vaccine is recommended yearly for everyone 6 months and older.

Meningococcal VIS

Meningitis vaccine is recommended at age 11 years and a booster at age 16 years.

HPV (Human Papillomavirus) Gardasil® VIS

HPV vaccine (a three dose series) is recommended starting at age 11 years for both boys and girls.
